What to do when someone dies.
When someone passes away, it can feel overwhelming and difficult to know what to do next. There are some practical steps that need to be taken. This guide is here to offer clear, simple advice and reassurance during a very difficult time.

What to do immediately
If the death happens at home.
Providing the death was expected and has been verified by a District Nurse or Medical Practitioner you can contact us. We provide a 24-hour service, so feel assured that your call will be personally answered. We will attend the home at a time convenient to you and convey your loved one into our care.
Expected
Contact the deceased's GP or district nurse immediately.
Unexpected
Call 999 and ask for an ambulance. A medical professional will confirm the death and advise on next steps.
If the death happens in hospital, hospice or care home.
The staff will take care of the immediate formalities and will explain what happens next.
We will take care of the paperwork to ensure your loved one is brought into our care with compassion and respect.
If the Coroner is needed we will explain the process and liaise with them, supporting you with care and understanding.
Registering a Death
You will receive a telephone call from the Medical Examiner Office before the Medical Certificate of the Cause of Death is issued. They cover all deaths in England and Wales that are not investigated by the coroner. A Medical Examiner is an independent Doctor that has not been involved in your loved one’s care. They speak with the treating Doctor to discuss the deceased’s care and the cause of death. They ensure that the information written on the Medical Certificate of Cause of Death (MCCD) is correct, that any deaths that need to go to the coroner are made in a timely fashion to avoid any delays, and that the bereaved have a chance to ask questions about the cause of death and reflect on the care provided. They will let you know whether you can contact the registrar to make an appointment to register the death. You cannot make an appointment until the registrar is in receipt of the Medical Certificate of Cause of Death (MCCD). After confirmation from the Medical Examiner’s Office, an appointment with your local registrar must be made within five days. This ensures the death is officially registered and that you receive the necessary documents. A death must be registered in the district office of the county where the death happened. If you live elsewhere, some offices allow registration by declaration. Please note, this should be completed within five days of confirmation by the medical examiner.

The Death Certificate
Ensure that you ask for multiple copies of this to allow you to send it to different organisations that need notifying about the death.
Certificate of Burial or Cremation
Also known as a ‘Green Form’, it gives permission for burial or cremation. Needed by a funeral director to send to crematoriums or burial authorities.
Certificate of Registration of Death
Also known as a ‘BD8 Form’, you may need to fill this out if the person was getting a State Pension or benefits. The form will come with a pre-paid envelope, so you know where to send it.

The registrar will also provide you with information about the relevant governeent orgnaisations and departments, such as the ‘Tell Us Once’ service online gov.uk service.

The Forest of Dean office, Cinderford, is housed in the Belle Vue Centre, formerly an imposing private Victorian residence for a wealthy local mine owner in the late 1850’s.
Forest of Dean Registration Office (The Oaks) has a maximum occupancy of up to 44 people to include 2 registration officers and no more than 40 guests.
The registration office has a car park adjacent and disabled access is at the side of the building. The main entrance is reached by a wide flight of stone steps, and a stone pathway leads to a secluded garden area, both are ideal for photographs. Disabled access available.
